Colorado Community Health Alliance (CCHA) is the go-between for Colorado’s Medicaid program for people who live in Boulder, Broomfield, Clear Creek, El Paso, Gilpin, Jefferson, Park or Teller County. When you call, you’ll be asked for your Medicaid number (some of you may have a card but these are no longer mailed out). If you can’t remember the number, they can look you up by your name, date of birth, and possibly your Social Security number. Operators will help you find the doctors you need (primary care for yearly physicals and chronic conditions, dental and vision, urgent care, mental health, substance use) and connections to other resources.
