Axis Health System has seven locations that provide care for people who live in La Plata County. Click on the website link, then click on the location near you. You'll see each health center's hours, service information, and the number to call to make an appointment. Axis also has a 24/7 crisis care hotline you can call after work hours at 970.247.5245 for a mental health or substance use crisis situation. You will be quickly connected to local services and a team will respond immediately. Your crisis may be resolved through this phone call, or they may ask to meet with you. During business hours, call Crossroads at Grandview at 970-403-0180 for help during a crisis.

Colorado’s Rural Clinics and Safety Net Clinics

Besides the community health centers listed in this section on remerg.com, Colorado also has clinics that are called Safety Net Clinics and Rural Health Clinics. These clinics are also an option for health care. Click on the website link to Colorado Health Institute and scroll down the page until you see a map. This map is color coded for all health care in Colorado. Rural health centers are located where you see a blue dot. Safety net clinics are located where you see a purple dot. You'll also see where community health centers are located (turquoise dot), as well as other health services. This is a good way to visualize the kinds of care available in Colorado and where they're located. We'll be adding more information as soon as it's available.

Denver Health’s Community Health Services provides care for people who live in the city and county of Denver. We list the location and hours of their main campus location, but they have MANY other locations in the Denver metro area where you will also be seen. Ask about their Community Voices Patient Navigator program if you would like help figuring out where to get an appointment. If you need immediate help and it’s not a life-threatening emergency, Denver Health’s Federico F. Pena Southwest Family Health Center and Urgent Care and Denver Health Adult Urgent Care Center provide urgent care for adults and children. You may come in without an appointment. They suggest you contact the Nurse Line at 303-739-1211 before seeking care. See Help for more information about urgent care and convenient care clinics. The Nurse Line at 303-739-1211 is also available for Denver Health patients and people who live in Denver. It's answered 24/7 and nurses will provide medical advice.

After hours, you can call 719-336-0261 and ask to speak to one of their physicians. Look on the website link for the direct numbers for the High Plains doctors. You can call for same day appointments when the phones are answered starting at 7:45am M-Sat. No appointment is necessary for an acute illness.

Colorado Coalition for the Homeless health centers provide integrated health care for people who are homeless or at risk of becoming homeless in the Denver metro area. New patients to Stout Street Health Center are asked to come in person on Monday, Tuesday, Thursday, or Friday between 7:00 am and 11:00 am, and on Wednesday at 10:30 am.
