Axis Health System has seven locations that provide care for people who live in La Plata County. Click on the website link, then click on the location near you. You'll see each health center's hours, service information, and the number to call to make an appointment. Axis also has a 24/7 crisis care hotline you can call after work hours at 970.247.5245 for a mental health or substance use crisis situation. You will be quickly connected to local services and a team will respond immediately. Your crisis may be resolved through this phone call, or they may ask to meet with you. During business hours, call Crossroads at Grandview at 970-403-0180 for help during a crisis.

Besides the community health centers listed in this section on remerg.com, Colorado also has clinics that are called Safety Net Clinics and Rural Health Clinics. These clinics are also an option for health care. Click on the website link to Colorado Health Institute and scroll down the page until you see a map. This map is color coded for all health care in Colorado. Rural health centers are located where you see a blue dot. Safety net clinics are located where you see a purple dot. You'll also see where community health centers are located (turquoise dot), as well as other health services. This is a good way to visualize the kinds of care available in Colorado and where they're located. We'll be adding more information as soon as it's available.

Denver Health’s Community Health Services provides care for people who live in the city and county of Denver. We list the location and hours of their main campus location, but they have MANY other locations in the Denver metro area where you will also be seen. Ask about their Community Voices Patient Navigator program if you would like help figuring out where to get an appointment. If you need immediate help and it’s not a life-threatening emergency, Denver Health’s Federico F. Pena Southwest Family Health Center and Urgent Care and Denver Health Adult Urgent Care Center provide urgent care for adults and children. You may come in without an appointment. They suggest you contact the Nurse Line at 303-739-1211 before seeking care. See Help for more information about urgent care and convenient care clinics. The Nurse Line at 303-739-1211 is also available for Denver Health patients and people who live in Denver. It's answered 24/7 and nurses will provide medical advice.

High Plains Community Health Center has six locations that provide care for people who live in Baca, Cheyenne, Kiowa, Kit Carson, and Prowers counties. Click on the website for locations and hours, including when you can walk-in without an appointment (called walk-in sick call). A High Plains doctor is ALWAYS on call. After hours, you can call 719-336-0261 and ask to speak to one of their physicians. Look on the website link for the direct numbers for the High Plains doctors. You can call for same day appointments when the phones are answered starting at 7:45am M-Sat. No appointment is necessary for an acute illness.

Stout Street Health Center is their largest health clinic, but there are other locations. Click on the website for information about each location and hours. New patients to Stout Street Health Center are asked to come in person on Monday, Tuesday, Thursday, or Friday between 7:00 am and 11:00 am, and on Wednesday at 10:30 am. Call about other locations and how they accept new patients. You are eligible to receive care at Stout Street Health Center if you're experiencing homelessness, living on the street, in shelters, or “doubled-up” with family/friends, you're at-risk of becoming homeless, living in any unstable or non-permanent situation, you're currently receiving services or housing through the Colorado Coalition for the Homeless, or you have chosen Stout Street Health Center as your Medicaid medical home.

STRIDE Community Health Center has twenty locations that provide care for people who live in Adams, Arapahoe, Douglas, Jefferson, and Park counties. (STRIDE was formerly known as MCPN). STRIDE has three health centers that provide Convenient Care, meaning that you can walk in and be seen for a sudden illness such as a sore throat, ear pain, rash, cough/asthma, headache, vomiting, diarrhea, or cold & flu symptoms, injuries such as a fall, cut, bruise, sprain, strain, or broken bones, infections such as a bladder infection, skin infection, or needed testing for a sexually transmitted infection, flu shots, immunizations, INR check, pregnancy test, and Depo injection. The clinics are in North Aurora, South Aurora, and Wheat Ridge. STRIDE clinics will see folks who are not eligible for insurance due to immigration status for medical visits and get them medicine. HIV medications are always free for people who make less than $63,800/year if they apply for the State Drug Assistance Program. See this section for more info about SDAP. Care is on a sliding scale, with most people paying around $15-20 for a visit and lab work.
